I & Y Seniorcare offers you the opportunity to fully manage your home care through the CDPAP program. This program gives you the ability to select your own caregiver.
Supported by New York Medicaid, this program empowers you with complete control over who you want taking care of you.
Here's the best news, your chosen caregiver, known as a Personal Assistant (PA), can be a family member, friend, or neighbor who completely understands your needs.
With CDPAP, I & Y Seniorcare ensures that the caregiver you choose is paid for their committed service, emphasizing the importance of customized care in the comfort of your own home.

Am I Eligible for CDPAP?
Yes, you are eligible for CDPAP as long as you
- Live in New York State
- Are eligible for New York State Medicaid*
- Has a stable medical condition
- Require assistance with ADL's (Activities of Daily Living)
- Be eligible for long-term care
Who is eligible to be a caregiver for CDPAP?
Yes, they are eligible for CDPAP as long as they

- Relatives
- Friends
- Must be at least 18 years old and be able to work in the US legally
